Reason:

Creation of Item Procedure procedure_name related to item_name item_version failed due to:

Name conflict, another User may have saved another Item Procedure at the same time.
Unknown unrecoverable error (the Procedure registration information is lost).

Action:

Check the Item Procedure list for the name: procedure_name. If not found, contact your systems support engineer and supply the following information:
