Reason:
Attempt to register (through distribution) an Item with Product name/version item_name item_version but this Product name/version or the Item's Item name/version are already used by some other Item in the Library. This is interpreted as this Item is already in the library.
Action:
Check if the item already in the library is the same as the one you are trying to register, through distribution.
