If you find no discrepancies between your procedures and the documented procedures, repeat the activity under conditions similar to those that existed when the problem first appeared. (If you no longer get unsatisfactory results, an inadvertent error may have caused the problem.)
If the same error recurs when you repeat a given activity, and you can find nothing in the documentation to suggest that your procedure may be flawed, try to secure assistance in resolving the problem from others at your site.
