Reason:
An attempt to allocate data set (ddd) failed with a return code indicating that the data set already exists on the target volume. An attempt was then made to retrieve the existing data set's VTOC entry, but this also failed. This is an extremely unusual condition. The DASD volume involved should be examined for possible VTOC damage or contamination. If there is no damage, try to process the data set again. Otherwise, select a different target volume and retry.
