I/O ERROR ON vaddr, STATUS=status, SENSE=sense

Reason:

While formatting the IPLDISK minidisk with the VMXIPL utility, a permanent I/O error occurred while writing on the specified minidisk. The CSW status and the first four bytes are displayed. The utility program terminates with return code 100.

Action:

Bring the error to the attention of the appropriate person in your installation. This message indicates a probable hardware error.
