Reason:
An attempt to add a new response to the application has failed because the number of responses already added is equal to the maximum responses entered on the Application Definition screen. The maximum responses limit was set too low. Return to the Application Definition screen and increase the maximum responses limit. Note, however, that this number should be kept as small as possible due to performance considerations.
