Reason:
The value entered in the MAXIMUM RESPONSES field on page 1 of the Application General Options screen is zero which means that the application cannot contain any responses. Therefore, no responses may be added using the Response Definition screen. This is probably a user error. Enter a nonzero MAXIMUM RESPONSES count on page 1 of the Application General Options screen.
