Reason:

The CVAFDSM macro was issued to get the free space extents on an indexed VTOC for volume (vvv), but this request failed. CADisk will continue with the next entry.

• RC=8 - Invalid VTOC index structure.
• RC=12 - The CVAF parameter list is not in your protect key or is invalid.
• RC=16 - An I/O error was encountered.
